  • Gerontology is a specialization in the care of older persons that provides the following services:

    • Gerontology

    • Intravenous therapy

    • Dressing changes

    • Pain & symptom management

    • Chronic disease management

    • Serious injury care

    • Post surgical care

    • Gerontology

    • Safety assessments

    • Collaboration with additional care providers

    • Provide detailed reports regarding the client to the family

    • Provide ongoing monitoring of health and well being

    • Assist client and families with transition in care (move to facility, increased care from RN, monitoring declining health and navigating through the health care system) ensuring that the journey is personalized for the client

  • A certified healthcare worker can provide the following services:

    • Personalized medication reminders

    • Assist with maintaining regular mobility

    • Monitoring patients and performing basic health checks

    • Washing and dressing

    • Preparing and serving meals

    • Helping to feed clients

    • Follow the pre—established care plan set out upon assessment by the geriatric nurse.

    • Providing detailed reports to the nurse manager

    • Maintaining the physical, emotional, cognitive & social wellbeing of clients

    • Periodic supervision by the geriatric nurse

  • An In-home & community worker can provide the following services either in home or within long term care & assisted living facilities.

    • Provide non-medical day to day assistance

    • Assist with transportation

    • Accompany the client (or provide the client) with grocery shopping assistance and other daily activities

    • Prepare and serve nutritious meals

    • Provide basic cleaning services

    • Promote health by adhering to the medication, diet and exercise plans set out by the geriatric nurse upon assessment and ongoing as needs change

    • Provide medication reminders

    • Periodic supervision by the geriatric nurse

  • Love Me Tender’s Registered Nurse Care Manager will schedule a home visit to evaluate just how effectively you or your loved one is coping. The goal for our client is to remain independent in their home by addressing physical and mental health conditions and assessing unmet needs.

    The registered nurse care manager will be able to proactively assess and detect health concerns and implement support to avoid unnecessary suffering and visits to the emergency room. We take a holistic approach when assessing and creating a care plan. This means we provide exceptional care and support that looks at the whole person, supporting their physical, emotional, social and spiritual wellbeing.

    Love Me Tender’s ongoing proactive approach is appreciated by the clients loved one(s) who cannot always be there. Often the client does not share their struggles and/or health concerns as they do not want to worry or inconvenience their busy families. They may not share as they fear being moved into a retirement residence, losing their sense of independence and leaving the comfort of their homes.

    There are three care level packages available and each begins with an intake assessment . This thorough assessment ensures complex and long-term health care clients receive the ongoing medical treatments they require for having a positive quality of life. The assessment will be designed to provide the level of support that addresses your concerns and can be adjusted to adapt to the changing needs of your loved one.
